AzAHQ Presents Perspectives in Transitions of Care

This session will summarize why ongoing collaboration between acute and post-acute care settings is important to ensure effective communication to identify common drivers for readmissions, including conditions most frequently related to readmissions, such as sepsis and heart failure. The session will close with sharing tools and resources to assist your organization in your care coordination and quality improvement efforts.

 

OBJECTIVES: 

1.  Participants will be able to summarize why collaboration between acute and post-acute care is important.       

2. Participants will be able to list the most common readmitted conditions.

3. Participants will be able to demonstrate competence with Teach-Back.

4. Participants will be able to use the tools reviewed in this presentation.
