  This session will summarize why ongoing collaboration between acute and 
 post-acute care settings is important to ensure effective communication to
  identify common drivers for readmissions\, including conditions most 
 frequently related to readmissions\, such as sepsis and heart failure. The
  session will close with sharing tools and resources to assist your 
 organization in your care coordination and quality improvement efforts.
