  This presentation will equip hospital leaders with a strategic framework 
 for implementing and scaling an Age-Friendly Health System. Using the 
 evidence-based &ldquo\;4-M&rsquo\;s&rdquo\; model &ndash\; What Matters\, 
 Medication\, Mentation\, and Mobility- attendees will learn how to drive a
  hospital-wide transformation that addresses the unique needs of older 
 adults. The session will cover leveraging executive buy-in to empower 
 frontline interdisciplinary teams\, to focus on what truly matters to 
 patients\, to foster a culture of person-centered care that improves 
 quality\, enhances safety\, and aligns with emerging quality measures from
  regulatory agencies. Learning Objectives Describe the Age-Friendly Health
  Systems 4Ms framework and how it applies to the five domains of the 
 measure Identify key stakeholders&rsquo\; imperative to advancing Age-
 Friendly care. Discuss integration of Age-Friendly care strategies in the 
 acute care setting. About the Speaker: Rita A. Stockman\, RN\, MS\, BSN\, 
 CSSBB\, CHSP\, CPHQ Rita is a registered nurse\, healthcare executive\, 
 safety leader\, and system director of quality at Bon Secours Mercy 
 Health. Before BSMH she was an Associate Vice President at UC Health\, 
 Cincinnati\, and system director for accreditation and regulatory services
  at Trinity Health. Her record of success includes roles in Catholic 
 Health systems and academic medical centers. She holds a Bachelor of 
 Science in Nursing from Madonna University and a Master of Science in 
 Administration from Central Michigan University. Rita has achieved 
 nationally recognized certifications in healthcare quality and safety and 
 is a Six Sigma Black Belt. Rita is chair of the Michigan Safety Conference
  Healthcare Division\, 2025- 2026 and Past President of the Michigan 
 Health Care Safety Association. This program has been submitted for 
 approval by NAHQ for 1.0 hour of CPHQ CE credit.
